IMPORTANT INFORMATION ABOUT YOUR APPOINTMENT

What forms will you need to fill out for a new patient appointment?

You will need to complete a medical health history, demographic and insurance form, sign our notice of privacy practices, and sign our office financial policy.

What do you need to bring to a new patient appointment?

Bring a copy of your insurance card (front and back), completed registration forms if you choose to completed them beforehand, a list of any medications you currently take, and a method of payment for your co-pay.

What insurance plans do we accept?

We accept most insurance plans. A list is available on this site. Check here. If you have questions about insurance coverage, please contact our office or your insurance plan directly.

Are podiatry services covered by insurance?

Yes. Podiatry falls under the same umbrella of medical services as other specialties. Some restrictions in services and supplies may be enforced by your insurance plan. For details, please contact them directly.

Where are we located?

Our office is conveniently located in West Jordan, on 7000 South, just East of Redwood Road. Look for a 2 story medical building behind Smith’s Food and Drug on the South side of 7000 South. You’ll find us on the first floor in Suite 100.

When should you arrive for a new appointment?

You should arrive 10-15 minutes before your scheduled appointment time. Be prepared to complete the registration forms if you have not already done so. 

How long is the waiting time once you arrive for an appointment?

Dr. Williams generally runs on time, so the waiting time averages less than 10 minutes. Due to unforeseen circumstances, some patients require more intense care and the waiting times can vary. Our staff will make every effort to warn you when you arrive if the wait time is longer than expected.

How long does a new patient visit generally take?

Typically, new patient appointments take 30 minutes. This can vary depending on whether procedures need to be performed or if your medical conditions are complex and require more time. If you suspect the need for additional time with Dr. Williams, then please let the receptionist know so we can schedule time accordingly.

How long does an established patient visit generally take?

Established patient visits typically take 20 minutes, depending on the nature of your medical problems. Patients needing office based procedures should request a longer appointment time. If you suspect the need for additional time with Dr. Williams, please let the receptionist know so we can afford you the necessary time.

What services can we provide here in the office?

Our office is equipped to provide most services typical in a medical office. We have on-site x-ray, ultrasound, DEXA scan, and we offer basic in-office minor procedures.  We also offer a revolutionary Gait Analysis that change the way you think about your movement.

Can you bring family members to your appointment?

We are happy to provide a comfortable environment for our patients and their family. To that end, we ask that you limit the number of guests at your appointment so we can focus on your medical care. 

Are you taking Self Pay patients?

We pride ourselves on putting our patients and their needs first. We do have options for self pay. Please inquire in office. We will have a published list for your convenience, coming soon!
